Carotid sinus massage in patients who fall: will it define the role of pacing?

چکیده

In this era of diagnostic and therapeutic glitz, can a simple, safe, and universally applicable clinical maneuver—carotid sinus massage—favorably affect a clinical outcome in a large number of patients? Possibly. In this issue of the Journal, Kenny et al. (1) have evaluated patients presenting to an emergency room because of an unexplained fall. In a prospective randomized study they assessed the effect of permanent pacing in individuals who had experienced nonaccidental falls and who had a positive cardioinhibitory response (a 3-s pause) to carotid sinus massage. Insertion of a dual-chamber pacemaker with rate-drop programming resulted in a 70% reduction in the number of subsequent falls compared to nonpaced controls and a 50% reduction in syncope. Given the consequences of falls in the elderly as reviewed by the investigators—injuries

Cardiac pacing reduced nonaccidental falls in older adults with cardioinhibitory carotid sinus hypersensitivity.

Patients 175 patients > 50 years of age (mean age 73 y, 59% women) with cardioinhibitory CSH who reported to the emergency department because of a non-accidental fall. Exclusion criteria included cognitive impairment (MiniMental State Examination score < 24), a medical cause for the fall within 10 days of presentation, an accidental fall, blindness, contraindication to carotid sinus massage (CS...

Carotid sinus syndrome: Progress in understanding and management

Carotid sinus syndrome (CSS) is a disease of the autonomic nervous system presenting with syncope, especially in older males who often have cardiovascular disease. The aetiology is unknown and epidemiological data is limited. Forty new patients/million population have been estimated to require pacing for CSS and these patients represent ∼9% of those presenting syncope to a specialist facility. ...

Carotid sinus syncope treated by pacing. Analysis of persistent symptoms and role of atrioventricular sequential pacing.

Seventy patients have been paced for carotid sinus syndrome over four years. Twelve patients had persistent symptoms despite adequate ventricular pacing. Patients with persistent symptoms were found to have a significant vasodepressor response, a significant hypotensive response to ventricular pacing (pacemaker effect), and a severe hypotensive response to carotid sinus massage with introductio...

The history of diagnosing carotid sinus hypersensitivity: why are the current criteria too sensitive?

The carotid sinus syndrome and carotid sinus hypersensitivity (CSH) are closely related disorders. The first is characterized by syncope triggered by manipulation of the carotid sinus in daily life (e.g. shaving). According to the current European Society of Cardiology guidelines, CSH is diagnosed when carotid sinus massage elicits ≥3 s asystole, a fall in systolic blood pressure of ≥50 mmHg, o...

Carotid sinus hypersensitivity: evaluation of the vasodepressor component.

The basis of the vasodepressor response in patients with carotid sinus hypersensitivity (CSH) is unknown, and prevention of recurrent vasodepressor-induced hypotension in these patients has not been possible. In this study we assessed the effectiveness of atrioventricular sequential pacing and pharmacologic interventions in the prevention of carotid sinus massage (CSM)-induced vasodepressor res...
